Works under the direction of the Home Medication Information RN to help provide indirect patient care by ensuring that the home medication information process is completed per policies and procedures. Acts as a resource to staff at the time of admission, the time of discharge, and when assistance is needed to obtain the best list possible of home medications.
Interacts collaboratively with the patient, family, staff, physicians, retail pharmacists, and any other resources needed to effectively complete the home medication process, assessing the optimal list of home medications, according to the National Patient Safety Standards.
Conduct interviews with patients and calls pharmacies, clinics, and doctor offices to get information related to medications when a patient is unclear on what medications to take.
Completes data entry of home medications into system of record.
Prioritizes patients to be seen based on triage level.
Maintains workspace by keeping the computer clean and rebooting the computer on a set schedule. Maintaining standards of cleanliness if the computer needs to be taken into an isolation room.
MINIMUM EDUCATION REQUIRED:
High School Diploma or GED Equivalent required.
MINIMUM EXPERIENCE REQUIRED:
One year of experience in a retail pharmacy setting required.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Registration (current) required with the state Board of Pharmacy as a Pharmacy Technician
