Job Summary: Accountable for the correct assessment and follow-up of patient's needs. Provides counseling, educational, information, and casework
services to patients and/or family members. Coordinates job activities with physicians, nursing personnel and other departments and
community agencies as necessary to assist with individualized patient social/emotional circumstances.
Two years' experience in long term care as a
Social Worker. One year administrative experience.
Education: Bachelor's Degree in Social Work, Human Services,
or Psychology. Bachelor's Degree in Social Work, Human Services, or Psychology. Master's preferred.
