Transplant Social Worker-LMSW

RESPONSIBLE FOR:
Provides in-depth psychosocial evaluationsand development of recommendations for potential liver transplant candidates, while working with medical and multidisciplinary liver transplant team. Overall tasks include psychosocial assessments, discharge planning coordination, and psychosocial interventions for high-risk populations, providing access to appropriate hospital and community resources for pre-liver and post-liver transplant candidates. The Liver Transplant Social Worker strives to promote patient and family wellness, recipient and donor advocacy and improved patient care outcomes. The Liver Transplant Social Worker supports and develops comprehensive transitional care plans for patients with complex health care needs in inpatient and outpatient settings.
MINIMUM EDUCATION REQUIRED:
Master's degree in Social Work (LMSW)
MINIMUM EXPERIENCE REQUIRED:
Two years of medical experience (hospital, nursing home, home health care, or dialysis)
Knowledge of Social Work processes with skills in counseling, case work, group work and community services.
Working knowledge of community resources and financial assistance programs.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
LMSW in the State of Georgia
ADDITIONAL QUALIFICATIONS:
Previous transplant experience or experience working with patients with end-stage organ disease preferred
