Social Worker MSW - Behavioral Health

The MSW provides psychosocial assessments, therapeutic interventions, and discharge planning for patients admitted to the inpatient behavioral health unit. This role collaborates with an interdisciplinary team to ensure continuity of care and safe transitions post-discharge.
Responsibilities:The Social Worker strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments.
- Conduct comprehensive psychosocial assessments and develop individualized care plans.
- Facilitate individual, group, and family therapy sessions as appropriate.
- Coordinate discharge planning, including referrals to community resources, outpatient services, and support programs.
- Advocate for patient needs and assist with crisis intervention when necessary.
- Maintain accurate documentation in compliance with regulatory and hospital standards
- Masters degree in Social Work (MSW) required; Licensed Masters Social Worker (LMSW) preferred.
- Experience in behavioral health settings and discharge planning strongly preferred.
- Strong communication, organizational, and collaboration skills.
