Shift/Schedule: PRN
JOB PURPOSE: Provides skilled speech and dysphagia therapy in the home setting to patients ranging in age from adolescent through geriatric. Responsible for the overall management of assigned patient case load, coordinating care with multiple disciplines, physicians and community resources to achieve highest level of functional swallowing and speech ability. Conducts speech therapy evaluations, interprets assessment data, plans, directs, and administers speech-language and swallowing therapy treatments as prescribed by the referring physician or as evident by the patient&rsquo#CD#s needs to restore function, prevent disability and help patients reach maximum potential. Facilitates consultations or diagnostic evaluations for patients having known or suspected communication disorders and/or oropharyngeal dysphagia. Practices in compliance with professional knowledge and standards of clinical practice and in accordance with policies and procedures of the agency, laws and regulations of Medicare, Medicaid and other third party payers. Demonstrates professional growth and accountability. Reports to and is supported by the Piedmont Home Health Clinical Manager.

MINIMUM EDUCATION REQUIRED:
Master&rsquo#CD#s Degree in Speech-Language Pathology/Communication Disorders from an accredited graduate school. Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA).
MINIMUM EXPERIENCE REQUIRED:
None.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Licensure by the Georgia Board of Examiners of Speech/Language Pathology. CPR certified. Valid Georgia Driver&rsquo#CD#s license.
ADDITIONAL QUALIFICATIONS:
Prior home health experience preferred. Knowledge of OASIS a plus.
