Care Manager

Overview:

Piedmont is seeking a Care Manager to help lead our Care Management team and drive exceptional patient outcomes across the continuum of care.

In this role, you'll have the opportunity to:

• Lead, coach, and mentor Care Management professionals
• Facilitate and prioritize complex workloads across the team
• Drive effective discharge planning and transitions of care
• Build strong partnerships with physicians, nursing leaders, payers, and community organizations
• Be part of Piedmont's innovative and evolving care management processes that are shaping the future of patient care

Responsibilities:

The Care Manager is responsible for care coordination, progression of care, and proactive discharge planning and is accountable for expediting the timely and safe discharge for all patients in their case load.

Qualifications:

• Graduate of Nursing Program Required
• Master's degree Master's degree Preferred

Work Experience

• 2 years of experience in an acute or post-acute setting Required
• Experience using InterQual or Milliman Preferred

Licenses and Certifications

• Current unrestricted registered nurse (RN) license, or eligible for licensure, in the state of Georgia Required

Additional Licenses and Certifications

• Certification in Case or Care Management Preferred
