Neuro Navigator

Responsibilities: The Neuroscience Navigator provides individual assistance to Neuroscience program patients and their families to overcome barriers to care, facilitate timely access to care, and provide education and support. The Neuroscience patient's trajectory through diagnosis, treatment, and aftercare survivorship will be seamless and coordinated with the multidisciplinary care team. The Neuroscience Navigator serves as the primary contact and advocate for patients, identifying barriers and having the knowledge to overcome those barriers, locating resources, coordinating services, and providing education. Supported programs include, but are not limited to, Stroke, Spine, and Cranial programs Qualifications: Education

• Graduate from a recognized, accredited school of nursing Required

• 2 years recent and relevant clinical experience related to Neuroscience programs. Required
• 5 years relevant clinical nursing experience Preferred
• 2 years leadership role; A leader who identifies opportunities for improved customer value; ability to build and support collegial relationships to achieve value-driven outcomes; self-starter Preferred

• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Upon Hire Required
