  • Provide expert consultation to referring clinicians and create clear co-management and return-to-referrer plans.
  • Partner with the Clinical Governance Council to establish standardized clinical protocols for blood-pressure measurement, referral triage, diagnostic workup, follow-up, medication titration, and urgent escalation.
  • Educate patients and families regarding blood-pressure goals, medication use, adherence, self-monitoring, diet, and long-term cardiovascular risk reduction.
  • Partner with administrative and clinical leaders to monitor access, quality, patient experience, clinical outcomes, utilization, and program growth.

  • Promote equitable, patient-centered access and approaches that address barriers to medication adherence, nutrition, monitoring, and follow-up.
  • Serve as the clinical champion and external-facing physician expert regarding hypertension on behalf of Piedmont Healthcare Institute.

QUALIFICATIONS
