Program Coord - Women's Heart Program
Overview: Responsible for developing, implementing and managing The Women's Heart Support Network in coordination with the Women's Heart Physician Team. Serves as point of contact for women with heart disease, inclusive of identifying and engaging women in the Piedmont Healthcare System as well as in the community. Promotes quality and engagement across the continuum of care to ensure high patient satisfaction. - Bachelors Degree in nursing, social work, or other healthcare-related Required
- 5 years of of experience in healthcare Required
- RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure based on degree Upon Hire Required or
- LCSW- License Clinical Social Worker based on degree Upon Hire Required
