Resource Coordinator

Overview: Responsible for working collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for all patients within our population health management population. Responsible for work under supervision of registered nurse and/or physician or advanced practice provider in coordinating self-management support and disease registry activities for the clinic's patient populations based on RN's clinical judgment, specific protocols, defined workflows and/or evidence-based guidelines. Qualifications: Education

  • H.S. Diploma or General Education Degree (GED) Required or
  • Graduate of a Medical Assistant program Required or
  • Nursing Assistant program (CNA) Required or
  • Licensed practicing nurse (LPN) Required or
  • Emergency medical technician (EMT); Required or
  • Other health-related field or counseling Required
Work Experience
  • 3 years of medical assistant in an outpatient clinic, patient care assistant, licensed practicing nurse, emergency medical technician, or a health coach in the health-related field or counseling Required
  • Health Coach Experience Preferred
Licenses and Certifications
  • Certified Medical Assistant (CMA)-AAMA from the Commission on Accreditation of Allied Health Education Programs (CAAHEP); or the Accrediting Bureau of Health Education Schools (ABHES); or a similarly recognized certifying organization. Upon Hire Required
  • Health coaching certification Preferred
